It is with deep regret that after twenty-seven years of operation, Potter Valley Community Health Center has been forced to close its doors. This decision was made as the inevitable result of the Governor’s recent decision to eliminate critical funding for uninsured patients seen by Potter Valley, coming on the heels of repeated rounds of cuts to State funding of health care services for rural and underserved patients. We wish to thank you for your loyalty, however, effective August 21, 2009, we closed our doors and will no longer be able to provide health care services to you or your family after that date. All medical and dental records are being stored at Mendocino Community Health Center, 333 Laws Avenue, Ukiah, CA 95482. If you need copies of your records, please call (707) 468-1010 and ask for “Records”. |
